Functional Capacity Assessment

An NDIS plan should accurately reflect a participant's lived reality. When an NDIS plan falls short, or when it is time for a scheduled plan reassessment, a Functional Capacity Assessment (FCA) is the most powerful piece of evidence you can provide.

At Pair2Care, our Social Work-led Functional Capacity Assessments look beyond physical limitations. We focus deeply on psychosocial functioning, emotional regulation, community safety, and systemic barriers to ensure participants receive the exact funding, core supports, and specialist services they need to thrive.

Why Choose a Social Work FCA?

While Occupational Therapists (OTs) evaluate physical environments and equipment needs, a Social Worker looks at the person within their entire social and systemic environment.

Our specialized assessments are ideal for participants navigating complex mental health conditions, psychosocial disabilities, neurodivergence, or trauma backgrounds.

How we compare

Traditional OT Assessment

  • Physical & mobility restrictions

  • Assistive technology & equipment

  • Home modifications and biomechanics

  • Fine Gross Motor skills

Social Work FCA Assessment covers:

  • Social Interaction & Community Inclusion: How mental health, communication barriers, or anxiety impact the ability to build relationships, access community, and participate in meaningful activities.

  • Self-Management, Safety & Vulnerability: Evaluating executive functioning, emotional regulation, risk of community exploitation, and the cognitive capacity to manage daily decision-making safely.

  • Self-Care & Daily Living: Assessing the person's independent ability to maintain personal hygiene, manage a household, prepare meals, and maintain a safe living routine.

  • Systemic Support & Advocacy Needs: An in-depth look at how the participant navigates complex housing, health, and legal systems, highlighting where formal funding is critical to prevent support breakdown.

Our comprehensive reports address the official NDIS functional domains, translating real-world struggles into clear clinical justifications.

Our Evidence-Based Assessment Framework

To ensure your report stands up to strict NDIS quality audits and planner scrutiny, our Social Workers don’t just rely on informal observations. We back our clinical findings with industry-standard, validated functional screening tools that are highly recognized by NDIS decision-makers. Depending on the participant's unique situation, we utilise:

  • WHODAS 2.0 (World Health Organization Disability Assessment Schedule)

  • LSP-16 (Life Skills Profile)

  • Vulnerability & Environmental Screening

Our Step by Step proces involves:

  1. Intake & Triage: We review current NDIS goals, past reports, and immediate funding risks to tailor our assessment focus.

  2. The Assessment Consultation: A comfortable, trauma-informed interview conducted face-to-face at home or your place of choice, or via telehealth. We speak directly with the participant and their preferred support network.

  3. Evidence-Based Report Writing: We compile a robust, audit-ready clinical report complete with standardized scales and psychosocial analysis.

  4. Actionable NDIS Recommendations: Your final report includes a clear framework directly linking our findings to specific NDIS budget lines, ensuring planners understand exactly why the funding is justified.

Frequently Asked Questions

Who can refer a participant for a Social Work FCA?

Anyone can initiate a referral! We accept direct inquiries from participants, family members, guardians, or community providers. Most of our referrals come directly from NDIS Support Coordinators preparing for a participant's upcoming plan reassurance or variation.

How much does a Functional Capacity Assessment cost?

Our assessments and comprehensive reporting are billed strictly in accordance with the current NDIS Support Catalogue pricing for Social Work services under the Capacity Building (Improved Daily Living) budget line. We will provide a transparent Service Agreement detailing the exact hours allocated for consultation, file review, and report formulation before work begins so there are no unexpected out-of-pocket costs.

What is your current turnaround time?

We understand that an impending NDIS review deadline can be incredibly stressful. Our standard turnaround time from your initial consultation to the delivery of the final audit-ready report is typically 2 to 4 weeks. If you have an urgent, tight deadline due to a sudden change of circumstances, please contact our intake team immediately so we can discuss fast-track availability.

Can the assessment be done online or in person?

We offer flexible delivery options to suit the participant’s comfort levels and anxiety thresholds. We can conduct assessments remotely or travel directly to the participant's home or preferred community venue.
